Prepare himself/herself for the procedure.

Follow instructions given by Consular Assistant for data capture.

Checks the official receipt issued to the client.

Encodes the applicant's personal information; scan pertinent documents; capture applicant's biometrics-- photo, signature and fingerprints. Assists applicant in the affixing of signature and fingerprint capture.

Prints the Enrolment Certificate and require the applicant to thoroughly check the information before signing said document.

Informs client on when is the tentative date of the arrival of his/her ePassport at Post and advise on how to claim his/her ePassport.

Inputs the fee paid by the passport applicant in the ePassport program.

Reviews the data encoded in the ePassport system and compare them to the submitted supporting documents; If the encoded data are accurate and documents are in order, the Signing Officer signs the application.

The epassport application is automatically sent, electronically, to the Office of Consular Affairs Passport Personalization Center as soon as the Signing Officer signs the enrolment certificate.

Proceed/return to the Embassy, present his/her original old passport for cancellation and receive his/her new passport. (FOR PERSONAL RELEASE).

Send old passport via cargo for cancellation and arrange for the cargo pick up of his/her new passport. (FOR CARGO RELEASE).

Informs client one-by-one, via phone or email, that his/her ePassport has arrived and shall be required to decide if he/she intends to claim his/her passports personally or via cargo.

Cancels the old passport as soon as it is received and dispatch it with the new passport via cargo.

In cases where visa applicants are Nationals of Azerbaijan or Foreigners/Expats residing in Azerbaijan and Georgia, visa requirements are sent via courier. For Georgian Nationals, they apply for their respective visas at the Philippine Honorary Consulate General in Tbilisi.

Ascertains the applicant's true identity and determines his/her Nationality/Citizenship; Assesses the applicant's request for visa; Verifies the applicant's name against the Department of Foreign Affairs and & Bureau of Immigration Blacklist.

For Non-restricted Nationals:

For a stay in the PHL of more than 30 days, the application shall be approved outright.

For restricted Nationals:

Application will be submitted for verification/clearance to the appropriate Philippine Government security agencies.

When the name of the visa applicant is cleared, his/her application will be approved.

Requests applicant to return to the Embassy for the payment and issuance of appropriate visa for entry to the Philippines.

  1.     Official Report of Marriage (EvlenmeKayıtÖrneği) issued from the City/Municipality Civil Registrar (NüfusMüdürlüğü)with english translation;

   2.     Photocopies of International/Multi-lingual (Red) Book of the Family (UluslararasıAileCüzdanı/Lıvret de Famılle  International) pages 1 to 7; and

  3.     Photocopy of passports/citizenship identifications of contracting parties.

   C)    REPORT OF DEATH and CONSULAR MOTUARY CERTIFICATE

    1.     Airway Bill and Flight details,

    2.     The name of the consignee or the name, address and contact no. of the person,  who is the authorized recipient of the cadaver in the Philippines,

    3.     A Certificate of Death issued by the local civil registrar, or similarly authority, showing the name, age and nationality of the deceased person, as well as the place, date and cause of death, as certified by the attending physician;

    4.     A duly notarized certificate issued by the Mortuary/Funeral Director, stating that the body was properly embalmed/cremated in accordance with internationally accepted health practices; that it is sanitary condition consistent with existing standards for overseas shipment; and that the casket/urn has been hermetically sealed and contains only the remains of the deceased;

    5.     A Certificate of Non-Contagious Disease issued by the proper health authority, stating that the deceased did not die of a contagious disease or that the remains to be shipped did not originate from a place contaminated by a contagious disease; and;  

    6.     A Transit Permit issued by the proper health authority at the place where transportation begins.

Note: Documents may be submitted personally or by courier (in case of applicants from Georgia and Azerbaijan).
